Infective endocarditis is a clinical condition frequently observed in individuals with intravenous drug use and is characteristically associated with right-sided heart valve involvement. In these patients, septic pulmonary emboli commonly lead to cavitary lesions in the lungs. In this report, we present a case of tricuspid valve infective endocarditis accompanied by septic pulmonary embolism in a patient with substance abuse. The case posed diagnostic challenges due to its resemblance to rheumatologic diseases. Through this presentation, we aim to emphasize the importance of considering infective endocarditis and septic embolism in the differential diagnosis of cavitary lung lesions in patients with intravenous drug use.
